Evidence and auditability describe the ability to reconstruct who acted, what changed, why it changed, and what supporting record exists without relying on undocumented memory.

Common Failure
A system that is explainable only by the team already inside it is not meaningfully reviewable. The record has to stand when the original operator is unavailable.
